Before filling out the requisition form, kindly read the following instructions:

The vehicle requisition must be submitted at least 24 hours in advance.
The user of the vehicle is expected to sign the logbook maintained in the vehicle.
The vehicle should not be detained beyond the period for which it has been requisitioned. If it is detained unexpectedly, the same must be communicated to the In-charge of Driver Duties.
Please ensure that you have all the necessary information ready before you begin filling out the requisition.
Upon successful submission, an acknowledgement email will be sent to the provided email address and additional email addresses.
